Study Title:
Changes in Spirituality and Religiosity Post-Amputation

Research Objectives:
The objective of the research study is to determining whether there is a change in belief systems (spiritual or religious) after a lower-limb amputation.
Eligibility Criteria:
Participant has a lower-limb amputation of a Syme’s level or higher, up to hip disarticulation.
Participant’s cause of amputation is non-congenital.
Participant’s amputation occurred at least 2 years prior to taking the survey.
Participant is 18 years of age or older.
Compensation Details:
No compensation.

Time Commitment:
The participant will take a one-time survey taking about 10 minutes.
